The proposed innovation is guided by the Triple Aim (Care, Health and Cost) framework that was developed in 2007 by the Institute of Healthcare Improvements (IHI). This framework is now globally adopted by multiple leading organizations, such as Canadian Foundation for Healthcare Improvements and the US National Quality Strategy . Its principle of emerging care design highlights the need of innovations via true collaborations between health users, workforce and communities. This strongly embraces the spirit of our proposed project. The framework emphasizes simultaneous efforts to improve the patient experience of care, improve the health of populations, and reduce the per capita cost of health care - Care, Health and Cost.
We have envisioned a multi-phase initiative that is focused on the innovation Co-Development (Year 1) and Evaluation (Year 2 and 3). A novel element is our plan for the Integrated Knowledge Translation (Year 2 and 3) and to organize two knowledge exchange events. We believe that York University and our partners provide highly conducive environment to successfully complete the proposed project.
